People within these diseases are often more susceptible to infection in general, the researchers explained, and are among the population’s most vulnerable to severe health outcomes from COVID-19 infections as well. The studies come as the U.S. total of COVID-19 deaths since the pandemic began surpassed 1 million.

Examining breakthrough infection rates in cancer patients

The first study, published recently in Journal of the American Medical Association (JAMA) Oncology, analyzed electronic health records to track the number of breakthrough COVID infections, hospitalizations and mortality rates among vaccinated patients with cancer.

A “breakthrough infection” is when a fully vaccinated person contracts COVID, according to the Centers for Disease Control and Prevention (CDC).

The research team counted people diagnosed with the 12 most common types of cancer: lung, breast, colorectal, bladder, liver, endometrial, skin, prostate, thyroid and blood cancers. These participants received COVID-19 vaccinations between December 2020 and November 2021 and had not previously been infected. The control group consisted of vaccinated participants without cancer.

The researchers compared breakthrough COVID-19 infections between cancer and non-cancer participants, matching for comorbidities, social determinants of health, age and gender, and other demographics. The team analyzed the records of more than 636,000 vaccinated patients, including more than 45,000 vaccinated people with cancer.

“This study showed significantly increased risks for COVID-19 breakthrough infection in vaccinated patients with cancer, especially those undergoing active cancer care, with marked variations among specific cancer types,” said Rong Xu, professor of biomedical informatics at the School of Medicine and coauthor of this study.

Study coauthors include David Kaelber, professor at the School of Medicine and chief medical informatics officer at The MetroHealth System.

The team found:

“Breakthrough infections in patients with cancer were associated with significant and substantial risks for hospitalizations and mortality. These results emphasize the need for patients with cancer to maintain mitigation practice, especially with the emergence of different virus variants and the waning immunity of vaccines,” said Xu and study coauthor, Nathan Berger, the Hanna-Payne Professor of Experimental Medicine at the School of Medicine.

Health outcomes in patients with Alzheimer’s disease following COVID infection

In a second, separate School of Medicine study, researchers analyzed electronic health data to examine the incidence rate of breakthrough COVID-19 infections in those diagnosed with some subtypes of dementia. The study was recently published in the journal, Alzheimer’s & Dementia.

The researchers chose to examine data on breakthough COVID-19 cases in those with dementia because, while vaccines are effective, breakthrough infections are possible, and older adults with dementia were underrepresented in vaccine clinical trials.

For example, in the clinical trial for Pfizer-BioNTech vaccine, the median age for the 37,706 participants was 52 years and included only 18 people with dementia, or just 0.05%, according to the study.

The researchers examined anonymous electronic health data from more than 262,847 adults 65 or older vaccinated between December 2020 and August 2021, and who didn’t have the infection before being vaccinated. Of that number, 2,764 people were diagnosed with Alzheimer’s disease; 1,244 with vascular dementia, 259 with Lewy body dementia, 229 with frontotemporal dementia and 4,385 with mild cognitive impairment.

The researchers compared the overall risks of breakthrough infections in vaccinated patients with dementia to those without any congitive impairment.

Vaccinated patients with dementia had an overall risk for breakthrough infections ranging from 10.3% for Alzheimer’s disease to 14.3% for Lewy body dementia, significantly higher than the 5.6% in the vaccinated older adults without dementia.

“Patients with dementia have a significantly higher rate of breakthrough COVID infections after vaccination than patients of the same age and risk factors other than dementia,” said Pamela Davis, the Arline H. and Curtis F. Garvin Research Professor at the School of Medicine. “Therefore, continued vigilance is needed, even after vaccination, to protect this vulnerable population. Caregivers should consider ongoing masking and social distancing, as well as booster vaccines to protect these individuals.”

Artists return to the stage and the streets while, little by little, life is redesigning itself and allowing us to remember the movement and energy that once fed cities. The Baiana System is one of those phenomena that, in order to fully occur, depends on the encounter, on human interaction, whether with the public or between musicians and creators of all kinds. Today we talked with Roberto Barreto, guitarist and creator of the group, about this scenario of recovery, the current moment of the group, Brazilian music, some dreams and about a close reunion with São Paulo, the city where the last Navio Pirata show took place before the earth stop.

2022 started with everything, what plans could you share with us for this year?

In fact, 2022 started postponing things a little, we thought that the movement would return in January, we had perspectives of some shows and also about carnival, anyway, some things there for January… But that was falling and, in fact, we’re just getting back to the show routine now. We did it in Rio and Salvador recently, a lot going on in April and May… And I think this year for all of us, in some way, is basically a year of return, of fresh start. For all the artists who recorded and released things during the pandemic, right.

We did some things in that period, we released an album, Oxe Axé Exú (listen here), a documentary – the “Manifestação: Carnaval do Invisível”, the album “Futuro DUB” with Buguinha Dub, and a live album as well, the “Gil Baiana”. The shows themselves were stopped, the Sulamericano Show, and now we start to return. I think the great plan and expectation of all of us is this resumption of face-to-face, contact with the public, playing new things in the show, relearning to be on stage with the exchanges and all the things that happen… with the commitments, the new ones and those that were pending because of the pandemic.

How is the relationship with the transformations and consequences of the pandemic and the current scenario of restructuring the culture and entertainment sector?

The main changes happened mainly because of the situation imposed by the pandemic, this non-presence thing influenced our entire production process. And now that we’re understanding what it’s like to play again, the new codes, how they’re happening, along with the audience, how the show behaves. It is a time for us to understand this together.

And Salvador, how is it musically after these 2 years?

We are not able to understand much because the shows and scenes are being redesigned now, for 1 month now… There was no carnival, the resumption is actually happening in April. I think everyone is hoping, to understand how it will be, there will be a lot by the end of the year, but for now everything is very uncertain.

How do you perceive this idea of ​​rescheduling Carnival to April? How is it over there?

I think it’s weird, right, carnival here, at least for us, is not just a party where you can reschedule the date/postpone and that’s it, it has a much broader meaning, it has Ash Wednesday, the relationship with the holy week, the spaces of the city, where it happens, like in the old center… So this is what carnival is, but other than that it can be a party that people want to call carnival because there was no carnival, but I don’t understand that with carnival.

How does this scenario impact the band’s creative process?

What I think has the most impact is precisely the contacts, the face-to-face contact, which happened, in every way, in a more effective way… From the moment of the show, the contact with the musicians, with the public, the contact in the studio, in the way of producing, this thing of the physical presence, of the real exchange, of you being really playing an instrument together with another person and being able to perceive how it impacts you directly, in real time, you know? And the show, the same thing, you play that song that you rehearse in a certain way, you imagine it’s in a certain way, but when you play it, it happens from the interaction with the audience, so the biggest impact was on that, and now that the people start to have a return of this heat, of the real exchange, and I think that this is showing up, returning to be more present in art in a more direct way.

One of the outstanding features of the group’s work is the way they combine elements from different musical universes. What artists/projects have you been inspiring at the moment?

We had already done this in Oxe, Axé, Exú, but we heard a lot from Latin America again, more recent and older productions as well. I haven’t been listening to much in this period because we just went back to rehearsing, meeting, traveling and everything, and with that a little bit of that is lost, right, this time of listening to other things… But let me think… There’s an artist that caught my attention recently, because I separate some things that I listen to on the African music program I produce, and I’ve been listening to a lot of Asa, a French singer from Nigeria.

Taking advantage of the fact that we are talking about music, how do you perceive the music scene in Brazil today?

One thing that I think artists are learning to deal with is the issue of numbers in this new market that has been configured with the internet, understanding what these numbers actually are, what these listeners are… The way of launching, of publishing … And with this story of not having a show, we had to deal a lot with other ways for the public to interact with us, right? I have observed this movement a lot, a greater understanding of this process, of understanding who we are talking to, how our work is distributed, at the same time I realize the importance of not getting stuck in this, simply in numbers and market analysis , in a coldly bureaucratic move. It is important to understand the other extensions of this work, the production process, the exchanges with the people with whom we make the music…

In July the band will be in São Paulo for a performance at the first edition of the Turá Festival, at Ibirapuera. What can we expect from this reunion with the capital of São Paulo?

It’s our return to São Paulo, right? Our last performance in São Paulo was at the 2020 carnival and it’s the memory we have, the last memory before the pandemic, all those people, the pirate ship… And soon after everything changed. So now being able to return to São Paulo, at a big festival like this one, in Ibirapuera Park, generates great expectations for us. We have the South American show “Brasiliana” version, which is what we are calling it and it is precisely the South American Show with some elements of the new album, so that’s it, the expectation is to show what we have best at this time. Anxiety for this reunion!

In 2022, the culture and entertainment sectors began to experience some respite from the isolation and social distancing imposed by the pandemic and with that, several events, meetings and festivals announced in-person editions. Artists return to the stage and the streets while, little by little, life is redesigning itself and allowing us to remember the movement and energy that once fed cities. The Baiana System is one of those phenomena that, in order to fully occur, depends on the encounter, on human interaction, whether with the public or between musicians and creators of all kinds. Today we talked with Roberto Barreto, guitarist and creator of the group, about this scenario of recovery, the current moment of the group, Brazilian music, some dreams and about a close reunion with São Paulo, the city where the last Navio Pirata show took place before the earth stop.

2022 started with everything, what plans could you share with us for this year?

In fact, 2022 started postponing things a little, we thought that the movement would return in January, we had perspectives of some shows and also about carnival, anyway, some things there for January… But that was falling and, in fact, we’re just getting back to the show routine now. We did it in Rio and Salvador recently, a lot going on in April and May… And I think this year for all of us, in some way, is basically a year of return, of fresh start. For all the artists who recorded and released things during the pandemic, right.

We did some things in that period, we released an album, Oxe Axé Exú (listen here), a documentary – the “Manifestação: Carnaval do Invisível”, the album “Futuro DUB” with Buguinha Dub, and a live album as well, the “Gil Baiana”. The shows themselves were stopped, the Sulamericano Show, and now we start to return. I think the great plan and expectation of all of us is this resumption of face-to-face, contact with the public, playing new things in the show, relearning to be on stage with the exchanges and all the things that happen… with the commitments, the new ones and those that were pending because of the pandemic.

How is the relationship with the transformations and consequences of the pandemic and the current scenario of restructuring the culture and entertainment sector?

The main changes happened mainly because of the situation imposed by the pandemic, this non-presence thing influenced our entire production process. And now that we’re understanding what it’s like to play again, the new codes, how they’re happening, along with the audience, how the show behaves. It is a time for us to understand this together.

And Salvador, how is it musically after these 2 years?

We are not able to understand much because the shows and scenes are being redesigned now, for 1 month now… There was no carnival, the resumption is actually happening in April. I think everyone is hoping, to understand how it will be, there will be a lot by the end of the year, but for now everything is very uncertain.

How do you perceive this idea of ​​rescheduling Carnival to April? How is it over there?

I think it’s weird, right, carnival here, at least for us, is not just a party where you can reschedule the date/postpone and that’s it, it has a much broader meaning, it has Ash Wednesday, the relationship with the holy week, the spaces of the city, where it happens, like in the old center… So this is what carnival is, but other than that it can be a party that people want to call carnival because there was no carnival, but I don’t understand that with carnival.

How does this scenario impact the band’s creative process?

What I think has the most impact is precisely the contacts, the face-to-face contact, which happened, in every way, in a more effective way… From the moment of the show, the contact with the musicians, with the public, the contact in the studio, in the way of producing, this thing of the physical presence, of the real exchange, of you being really playing an instrument together with another person and being able to perceive how it impacts you directly, in real time, you know? And the show, the same thing, you play that song that you rehearse in a certain way, you imagine it’s in a certain way, but when you play it, it happens from the interaction with the audience, so the biggest impact was on that, and now that the people start to have a return of this heat, of the real exchange, and I think that this is showing up, returning to be more present in art in a more direct way.

One of the outstanding features of the group’s work is the way they combine elements from different musical universes. What artists/projects have you been inspiring at the moment?

We had already done this in Oxe, Axé, Exú, but we heard a lot from Latin America again, more recent and older productions as well. I haven’t been listening to much in this period because we just went back to rehearsing, meeting, traveling and everything, and with that a little bit of that is lost, right, this time of listening to other things… But let me think… There’s an artist that caught my attention recently, because I separate some things that I listen to on the African music program I produce, and I’ve been listening to a lot of Asa, a French singer from Nigeria.

Taking advantage of the fact that we are talking about music, how do you perceive the music scene in Brazil today?

One thing that I think artists are learning to deal with is the issue of numbers in this new market that has been configured with the internet, understanding what these numbers actually are, what these listeners are… The way of launching, of publishing … And with this story of not having a show, we had to deal a lot with other ways for the public to interact with us, right? I have observed this movement a lot, a greater understanding of this process, of understanding who we are talking to, how our work is distributed, at the same time I realize the importance of not getting stuck in this, simply in numbers and market analysis , in a coldly bureaucratic move. It is important to understand the other extensions of this work, the production process, the exchanges with the people with whom we make the music…

In July the band will be in São Paulo for a performance at the first edition of the Turá Festival, at Ibirapuera. What can we expect from this reunion with the capital of São Paulo?

It’s our return to São Paulo, right? Our last performance in São Paulo was at the 2020 carnival and it’s the memory we have, the last memory before the pandemic, all those people, the pirate ship… And soon after everything changed. So now being able to return to São Paulo, at a big festival like this one, in Ibirapuera Park, generates great expectations for us. We have the South American show “Brasiliana” version, which is what we are calling it and it is precisely the South American Show with some elements of the new album, so that’s it, the expectation is to show what we have best at this time. Anxiety for this reunion!

At this moment? Change the current situation we live in, the politics of this country. If we can do that this year, we’ll be much better than we are now.

Gravitas Ventures, an Anthem Sports & Entertainment Company, has acquired the North American rights to distribute the award-winning Broadway-inspired queer millennial musical love story, “Triple Threat,” starring Stacey Maltin in her directorial debut and producing with co-star Margarita Zhitnikova under their Besties Make Movies banner.

The independent film, which co-stars Jay DeYonker (also co-writer), Mark St. Cyr (“The Menu,” “High School Musical”), Broadway’s Aramie Payton and Catherine Curtin (“Stranger Things”), features “a love story within a love story” and has been hailed as “a euphoric journey” with “high octane and sensual choreography…and soaring music.” (Broadway World) It will be released theatrically in select markets and on-demand on June 21, 2022.

The research also identified differences between clinician-based data and the patient-reported data, with some clinicians overestimating their patient’s physical wellbeing.

“An essential component of patient-centred care, shared decision making is a process in which the clinician and patient collate and discuss the available evidence on the benefits and harms of treatments, ensuring the most appropriate and informed decision is made for the patient,” says study lead author Natansh Modi, an NHMRC PhD candidate in the Clinical Cancer Epidemiology Lab at Flinders University.

Two tools can be used during shared-decision making: the Eastern Cooperative Oncology Group performance status (ECOG PS), a tool interpreted by the doctor, and patient-reported outcomes (PROs), structured tools whereby a patient self-reports their perspective on their physical, social, emotional, and functional abilities.

“PROs are generally used as secondary data in clinical trials to help with interpreting results; however, they have recently shown to be important in providing a prognosis to the patient for cancer types including bladder, lung and skin cancers, but their value to HER2-positive advanced breast cancer had yet to be fully explored,” says Mr Modi.

Published in the journal ESMO Open, the study pooled data from several trials to look at almost 3000 patients who underwent drug treatments for human epidermal growth factor receptor 2 (HER2)-positive breast cancer.

“We found that a number of patient-reported outcomes, including those on physical well-being and mental health, were identified as significant factors associated with either the patient’s overall survival of cancer, with cancer not progressing or severe adverse events during treatment,” says Mr Modi.

“The results indicate that information provided by the patients themselves was better able to predict their reaction to the treatment and their overall prognosis than the clinician-interpreted scores.

“Significantly, patient-reported physical well-being was found to be the most useful patient-reported outcome in determining a diagnosis for all available treatment outcomes.”

In some cases, the study found very different results between the clinician-based data and the patient-reported data.

“We determined that around 70% of the patients who their clinicians defined as ‘fully active, and able to carry on all pre-disease performance without restrictions’ went on to report limitations in their physical well-being status when asked to self-report,” says Mr Modi.

The authors say the research highlights the importance of listening to patients, with both types of tools providing helpful independent prognosis information.

“This study demonstrates that patient-reported physical well-being can predict prognosis better than clinician-interpreted ECOG PS. It is, therefore, essential that clinical practice transforms to place a greater emphasis on the patient’s perspective and voice,” says Mr Modi.

“By combining patient-reported questionnaires and clinically interpreted measures, we can provide the best clinical insights that allow for shared decision-making in cancer treatment, while also enhancing the design of future clinical trials.”

“An essential component of patient-centred care, shared decision making is a process in which the clinician and patient collate and discuss the available evidence on the benefits and harms of treatments, ensuring the most appropriate and informed decision is made for the patient,” says study lead author Natansh Modi, an NHMRC PhD candidate in the Clinical Cancer Epidemiology Lab at Flinders University.

Two tools can be used during shared-decision making: the Eastern Cooperative Oncology Group performance status (ECOG PS), a tool interpreted by the doctor, and patient-reported outcomes (PROs), structured tools whereby a patient self-reports their perspective on their physical, social, emotional, and functional abilities.

“PROs are generally used as secondary data in clinical trials to help with interpreting results; however, they have recently shown to be important in providing a prognosis to the patient for cancer types including bladder, lung and skin cancers, but their value to HER2-positive advanced breast cancer had yet to be fully explored,” says Mr Modi.

Published in the journal ESMO Open, the study pooled data from several trials to look at almost 3000 patients who underwent drug treatments for human epidermal growth factor receptor 2 (HER2)-positive breast cancer.

“We found that a number of patient-reported outcomes, including those on physical well-being and mental health, were identified as significant factors associated with either the patient’s overall survival of cancer, with cancer not progressing or severe adverse events during treatment,” says Mr Modi.

“The results indicate that information provided by the patients themselves was better able to predict their reaction to the treatment and their overall prognosis than the clinician-interpreted scores.

“Significantly, patient-reported physical well-being was found to be the most useful patient-reported outcome in determining a diagnosis for all available treatment outcomes.”

In some cases, the study found very different results between the clinician-based data and the patient-reported data.

“We determined that around 70% of the patients who their clinicians defined as ‘fully active, and able to carry on all pre-disease performance without restrictions’ went on to report limitations in their physical well-being status when asked to self-report,” says Mr Modi.

The authors say the research highlights the importance of listening to patients, with both types of tools providing helpful independent prognosis information.

“This study demonstrates that patient-reported physical well-being can predict prognosis better than clinician-interpreted ECOG PS. It is, therefore, essential that clinical practice transforms to place a greater emphasis on the patient’s perspective and voice,” says Mr Modi.

“By combining patient-reported questionnaires and clinically interpreted measures, we can provide the best clinical insights that allow for shared decision-making in cancer treatment, while also enhancing the design of future clinical trials.”
